How can I make an appointment with her new clinic?
There are three ways to make an appointment with Kallgren Dermatology Clinic:
- Call 303-444-8100 and request a time and date for an appointment.
- Email us at kallgrenderm@comcast.net and
request and date and time for an appointment.
- Request an appointment using our website.
How can I get my Medical Record transferred to her new clinic?
Go to Forms and Info, and
download a Medical Release form. Fax the form to the location of
the dermatology clinic you would like your records transferred
from and follow the instructions of the FAQ above to make an appointment
at Kallgren Dermatology Clinic.
Where is the new practice located?
The new clinic is located at 3434 47th Street Suite #110, Boulder, CO 80301.
This just east of Foothills Parkway at the corner of the
Diagonal Highway and 47th Street in the Northpoint Offices (also
known as the Colorado Group Building). Look at our map located
on our Contact Us page or Mapquest our address.
I had an appointment in the future with Dr. Kallgren at her former
practice. Does this mean my original appointment date and time
is the same?
Unfortunately, upon Dr. Kallgren’s departure from Boulder Dermatology
we had no way of knowing which patients would transfer their care
to her new practice and which patients would remain with her former
clinic. Therefore, the original appointment dates and times could not be
seamlessly transferred to her new clinic. We will make every attempt to
see patients as close to their original date and time as possible.
Does Dr. Kallgren’s new clinic accept the
same insurance programs as her former clinic?
At this time Dr. Kallgren accepts most of
the same insurance carriers as she did in the former clinic to
include Medicare, Aetna, Blue Cross/Blue Shield, Pacificare and
United Health Care. These include those insurance carriers listed
under Billing.
Does Dr. Kallgren accept Medicaid?
At this time Dr. Kallgren does not accept Medicaid.
What cosmetic procedures does her new clinic provide?
At this time Kallgren Dermatology Clinic performs both Chemical Peels
and Botox injections.
I was a patient of Dr. Kallgren's at her former clinic. Do I need to fill out paperwork for my initial appointment at her new practice?
No personal information including your address, phone numbers and insurnace data was transferred with your medical record. Because of this you will need to fill out a Medical History, Patient Registration and Policy and Procedures forms during your initial appointment.
Can I fill out the forms ahead of time?
Absolutely. Just down load the forms onto your computer and print them. Don't forget to bring the forms with you when you come to our office for your appointment (there's nothing worse than filling out forms twice).
